infographic with text that reads: The transition from high school to post-secondary education, employment, technical training and independent living can feel intimidating, uncertain and a little scary. Especially if you or your student is already in high school and graduation seems to be looming just around the corner! The earlier you start preparing for life after high school, the more confident and equipped both you and your child will be.       Infographic with text that reads: What is Transition Planning? Transition planning is a student-centered process designed to prepare youth with disabilities for life after high school. It includes setting goals and taking steps toward postsecondary education, employment, independent living, and community participation. Starting early helps students build skills and confidence, and ensures families have time to access the resources and supports their child needs for success.   infographic with text that reads: Whether your student is in elementary school or a senior in high school, let us take the guesswork out of the equation, start where you are, and help you and your student create a path towards independence - whatever that level that may be for them - We see you, we understand you and we are here to help! Our youth transition program is part of the Kansas Youth Transition Network and the Youth Transition Alliance of Wyandotte County.

Note: The Kansas Youth Transition Network (KYTN) was established under a grant from the Kansas Department of Education, and is comprised of Youth Transition Councils and Community Groups working together to support youth with disabilities and their families and caretakers throughout Kansas, in their journey to post-high school independence, employment or higher education. Independence, Inc. is working under the KYTN grant to create innovate ways of working with youth ages 10 and up through extended Youth Transition Services within Wyandotte County, as we partner with other agencies and school districts to support the independent living and employment goals of youth with disabilities.
