Disaster Response Questionnaire

Questions For Persons Experiencing the May 8 Tornado

During the Disaster:

  1. Where were you (location) during the disaster?

  2. What were your experiences during the disaster as either a family member of a person with a disability or as someone with a disability? (Identify the type of disability such as aging, hearing, mental, or mobility impairment)

  3. Where you able to be in a safe place for being in a tornado?

  4. What if any barriers did you encounter? 

  5. What did or did not work for you?

  6.  What changes could be made to make you feel safer in a disaster?


After the Disaster:

  1. What difficulties, if any, did you experience returning to everyday life after the disaster?

  2. What were your experiences with the police, hospital personnel, emergency management,  shelter, temporary housing after the disaster, the media, and your housing complex? 

  3. Was FEMA beneficial and in what ways for someone with a disability?

  4. What gaps in services did you encounter and how might these be corrected?
